At Rose Park School, we value the support of external service providers who deliver essential services to students, including therapeutic services under the National Disability Insurance Scheme (NDIS).
However, in line with the procedure for non-education service provision in preschools and schools, all external service providers must contact the school at the beginning of the year before commencing any appointments.
To ensure proper coordination, parents or guardians must complete the following forms before services can begin:
-
A parent/guardian request for non-education service provider form
-
A permission to exchange information form
https://forms.office.com/r/7mLdsuaUJJ
These forms must be approved by the Principal before the provider can start. Additionally, appointments must not interfere with the normal classroom program, and a suitable space must be available for sessions.
When considering requests for external service providers, the school will assess several factors, including the provider’s qualifications, insurance, necessary screenings, and training. The school must also ensure that staff maintain a clear ‘line of sight’ during appointments and that facilities are appropriate for the services being provided.
We encourage families or carers to reach out if they wish to request access for external providers. All requests will be reviewed to ensure they align with the school’s procedures and educational needs.
Thank you for your cooperation in ensuring a smooth and effective process for accessing external services.
